Reablement is a service that provides tailored support to people in their own home. It builds confidence and helps people to regain skills to live as independently and safely as possible.

An assessment is carried out by an occupational therapist, an assistant manager or an NHS rehabilitation clinician to agree on the goals the carers will work towards achieving with the service user.

The agreed goals can range from meal preparation to personal care, and the Reablement home carers will support the service users to achieve these goals.

Progress is continually reviewed, and care plans are updated in order to monitor and adapt to individual care needs for a period of up to six weeks.

After the final review, the Reablement team will look at what ongoing care, if any, is needed at home and will arrange for Cordia's home care service to provide this.
